Whether you have questions about a service or if you would like to schedule an appointment, we are here for you. You can call any one of our five Twin Cities locations to schedule an appointment at PDR. Just specify the location you would like to visit and we will do everything we can to get you scheduled as promptly as possible.
Prior to scheduling your first appointment, you may print your new patient forms directly from our website (see links below), or visit our Patient Portal. If you cannot access the paperwork, please contact us, and we will mail you the paperwork before your first appointment. It is essential that all of your paperwork is complete when you come in for your first visit.
During your initial visit to PDR, you will first meet with one of our Patient Representatives. They will review your completed paperwork as well as your health history and any medications you are currently taking. Please ensure that you bring a complete list of all of your medications and dosages with you to your first appointment. Next, you will meet with one of our spine specialists for your exam. Once you have completed your time with our doctor/physician assistant, you will have the opportunity to speak with our Accounts Receivable Specialists that will review our Financial Policy and assist you with any insurance concerns you may have.

New patient forms
If you are a new or returning patient, we provide your patient forms at no charge on our website. These forms can also be emailed to you or filled out in-person (just make sure to come 20 minutes early to your appointment).
