Our medical receptionist is available to schedule your appointments from 8:00 am - 4:30 pm Monday - Friday. Normal clinic hours are from 7:00 am to 7:00 pm Monday - Friday, and 8:00 am-12:00 pm Saturdays. Please refer to the list below for the clinic closest to you.

These forms are listed below. Please bring your completed forms with you to our office on your first visit. If you do not have a printer, we can send you these forms in the mail upon scheduling your first appointment.
