Thank you for your interest in Physicians’ Diagnostics & Rehabilitation. We try to make the process of scheduling your first appointment at PDR as simple and convenient as possible. Here are some general guidelines to make sure your first visit runs smoothly.

When scheduling your first visit, please be sure you have the following:

Health history forms
For your convenience we have made the necessary forms available for download/printing on our web site to make your initial visit as stress-free as possible. Please be sure to fill out all forms prior to your first appointment to minimize your wait time.

Photo Identification
Please bring in your drivers license or other state issued photo identification.

Insurance information
Please bring your insurance card with you to your first appointment, along with your completed forms. If you cannot locate your insurance card, you’ll need to provide us with your insurance carrier information and policy number prior to your first appointment.

Previous diagnostic studies
If you have undergone previous diagnostic studies such as MRI, CT, EMG, or a bone density test, and have the films or reports in your possession, please bring them to your first appointment for the doctor to review. If you have undergone previous studies and do not have them in your possession, please let your patient representative know so they can request them in advance of your first appointment. This will help ensure that your first visit goes smoothly and offers you the greatest benefit.
