Simply check whichever service you’re making a referral for (MedX rehab or Physical Therapy only), fax to us at one of the numbers listed at the bottom of the referral form, and we’ll take care of the rest! Our patient representatives will call for insurance verification and prior authorization, as well as call the patients to get them scheduled.
